You would like to service your MAS-100 instrument or have it calibrated at our headquarters in Staefa? Together with you, we ensure that your package reaches us quickly and reliably. Follow this step-by-step instruction. You will find all important documents attached. Please do not hesitate to contact us if you have any questions.

1. FILL IN ORDER FORM
Order form for instrument services contains all information you need to send your instrument to our service centre. This document also contains important information on logistics and customs formalities as well as warranty and repair services.
Download the order form via the following link, read it through at your leisure and fill out the form completely.
2. SUBMIT DECONTAMINATION DECLARATION
To ensure the safety and health of our service staff at all times, we require a fully completed and signed decontamination declaration from you before shipping your instrument to MBV. To do so, please send a scanned version of the decontamination declaration by email to our service center before shipping your parcel.
3. ENCLOSE template COMMERCIAL INVOICE
Place your instrument in the original transport case, pack it securely in a shipping box and enclose the completed order form and the original decontamination declaration with the package. If you are shipping your MAS-100 instrument outside Switzerland, we recommend that you enclose the following template commercial invoice for export customs clearance. This simplifies the import considerably and minimizes the risk of your package being held up at customs. Your package is now ready for shipment.
4. Check completeness
Please check again that all documents are complete before sending.

- Order form (filled in and signed)
- Decontamination declaration (filled in and signed)
- Sample commercial invoice (if shipped from outside Switzerland)
